What is a biometric screening?

A biometric screening is a short clinical measurement — typically 15 to 20 minutes per employee — that captures the numbers most predictive of long-term health cost and productivity. The CDC defines biometric screening as “the measurement of physical characteristics such as height, weight, body mass index, blood pressure, blood cholesterol, blood glucose, and aerobic fitness tests.”

For employers, screenings do three things at once: give each employee a personalized snapshot they can act on, feed a health risk assessment (HRA) that turns the numbers into next steps, and produce a de-identified population view that helps benefits teams design a health plan around the risks their workforce actually carries.

What we measure at every event

  • Height, weight, and body mass index (BMI)
  • Waist circumference
  • Resting blood pressure
  • Total cholesterol, HDL, LDL, and triglycerides
  • Blood glucose (fasting or non-fasting)
  • Optional HbA1c for diabetes risk
  • Optional hs-CRP for cardiovascular inflammation
  • Optional cotinine for tobacco cessation incentive programs

Each measurement produces a green / yellow / red result on the participant summary, aligned to established clinical guidelines from the ACC/AHA (cardiovascular), ADA (diabetes), and NIH (BMI/obesity).

How PicMed runs an onsite biometric screening event

We use the same 6- to 8-week planning window as our flu programs, with compressed 2- to 3-week delivery available when timing is tight.

Planning (Weeks 1–3)

Scope headcount per site, choose fingerstick or venipuncture, define incentive rules, integrate with your wellness platform or HRA vendor, and finalize the communications calendar.

Site walkthrough (Week 4)

Confirm privacy setup — screenings need a check-in area, private measurement stations, and a quiet space for results counseling. We provide a floor-plan template for each site.

Employee registration (Weeks 4–6)

Employees register in MyPicMed with your branding on the flow. Digital consent and HIPAA acknowledgment are captured at registration to keep event-day flow fast.

Event day (Week 7–8)

  • Check-in and verification
  • Vitals (height, weight, BMI, waist, blood pressure)
  • Blood collection (fingerstick or venipuncture)
  • Immediate results counseling for fingerstick events
  • Digital delivery of full report within 24 hours (fingerstick) or 3–5 business days (venipuncture)

Reporting and follow-up (Week 8+)

Aggregate dashboards published in MyPicMed. Incentive completion pushed to your benefits admin, payroll, or wellness platform.

Minimum onsite event size: no minimum. Smaller groups are best served by PSC + at-home options.

Fingerstick vs venipuncture

Both methods produce clinically valid results. The right choice depends on participation goals, budget, and how many measures you want.

Factor Fingerstick (point-of-care) Venipuncture (reference lab)
Time per participant 15–20 min 10–15 min (results later)
Results Immediate, counseled onsite 3–5 business days
Employee experience Slightly more perceived pain Familiar blood draw
Participation lift Highest — immediate results drive follow-through Slightly lower
Test breadth ~9 measures (lipid, glucose, HbA1c, hs-CRP, liver enzymes) 1,000+ available measures
Accuracy Clinically valid CLIA-waived POC Reference-lab gold standard
Best fit High-participation programs, incentive completion by event day Advanced panels, executive physicals, deeper HRA feeds

Most PicMed clients run fingerstick because participation rates are 15–25% higher on average. Employers who want advanced lipid particle-size testing, thyroid, hormone, or PSA panels choose venipuncture. Both methods can run at the same event.

For the full clinical comparison see our article on fingerstick vs venipuncture biometric screening.

Delivery options for every worker

  • Onsite events for concentrated worksites — our default
  • PSC network for remote employees near a national patient service center
  • At-home fingerstick kits for fully distributed teams or gap-closing after onsite events
  • Combined wellness weeks — biometrics plus flu shots plus HRA in one visit

Every delivery method rolls into the same MyPicMed aggregate dashboard, so benefits teams see one number for participation regardless of channel.

Reporting, HRA, and incentive tracking

MyPicMed portal. HIPAA-compliant reporting. Aggregate participation, biometric distribution, and risk-stratification for your workforce. Never individual PHI to the benefits team.

HRA integration. Biometric numbers auto-populate the HRA, producing a single personalized report for each employee. See health risk assessment for employers.

Incentive tracking. Two design patterns, both fully supported:

  • Participation-based — employee completes the screening, incentive earned. Simplest, EEOC-friendly.
  • Outcome-based — employee hits a target (e.g., non-tobacco, BMI under 30) or completes a reasonable alternative standard. Requires a documented alternative path per EEOC and ACA rules; we help you design it.

Completion data pushes to your benefits admin, payroll, or wellness platform on a schedule you set.

Wellness platform integrations. We deliver aggregate and de-identified data feeds to the platforms your team already uses, with a documented BAA in place.

Compliance and credentials

  • HIPAA — PHI encrypted in transit and at rest, role-based access, BAAs with every subprocessor
  • ADA — voluntary participation, reasonable accommodations available
  • GINA — no genetic or family-history data collected outside voluntary HRA sections
  • EEOC — incentive design reviewed against current wellness guidance
  • CLIA-waived point-of-care instruments for fingerstick components
  • CLIA-certified reference labs for all venipuncture components
  • W-2 clinical staff — every RN and LPN is a PicMed employee under our professional liability policy

Why employers choose PicMed for biometric screening

  • One project manager owns your program end to end
  • W-2 nurses on every event, in every state
  • Fingerstick and venipuncture available at the same event
  • Onsite + PSC + at-home blended coverage as standard
  • Compressed 2–3 week delivery when timing is tight
  • Live dashboard for benefits teams instead of a static PDF export

Frequently asked questions

What is included in a PicMed biometric screening? Every event captures height, weight, BMI, waist circumference, resting blood pressure, and a lipid + glucose panel via fingerstick or venipuncture. HbA1c, hs-CRP, and cotinine are available as options.

Fingerstick or venipuncture — which should we choose? Fingerstick drives higher participation because employees see results immediately. Venipuncture opens up 1,000+ additional test options. Many clients run both at the same event and let employees choose.

How much does a biometric screening cost per employee? Pricing starts at pricing available on request, with the final quote driven by headcount, sites, and geography. Request a quote for site-specific pricing.

Is a biometric screening HIPAA-protected? Yes. All PHI is encrypted in transit and at rest, access is role-based, and benefits teams only see de-identified aggregate data.

How is this different from an annual physical? A biometric screening is a fast, standardized snapshot focused on the highest-value risk markers. It is not a replacement for a primary care visit — it is a trigger for one. Our results reports flag any values needing follow-up and recommend a PCP visit when appropriate.

Do employees need to fast? A 9–12 hour fast improves LDL and triglyceride accuracy from venipuncture. Fingerstick lipid panels do not require fasting. We publish the fasting guidance in the registration flow.

Can screenings run alongside a flu clinic? Yes. Combined wellness weeks are our most common design. See onsite flu shot clinics.

How do you handle remote employees? PSC network appointments or shipped at-home fingerstick kits. Both channels feed the same MyPicMed dashboard.
